When you come to the office please remember to bring your auto insurance information with your claim number if you were involved in an auto accident, or your worker’s compensation claim number if you were injured at work.  It is important in both cases, that you report your injury to your auto insurance agency if it is a Motor Vehicle Accident (MVA), or to your Employer if it is a work related injury.

If you don’t have a claim number when you come for your initial visit don’t worry about it.  It is more important to seek medical attention to document your injuries.  Our staff can also help you with the paperwork process (Also look at Resources for more information)*

If you were referred to us for a 2nd opinion and you have been elsewhere for treatment it is important to bring a copy of records from previous physicians and any diagnostic study results.  It is also important to have a list of all the medications that you are currently taking.
